you can find some tips on how to create database using phpmyadmin on PHP forum in "toturial: guest book"

If you go to the MySQL page in your cPanel, near the bottom it there should be a text field with the words "New Database" next to it, type in the name you want the database to have, then **** "Create Database", and that's how you make a database.
You will also need to create a user for the database and assign it to the database, then use phpMyAdmin to edit the database.
